Introductory Call
Begin the enrollment process by filling out our online form. An experienced member of Five Star Home Care's team will reach out to discuss your eligibility and guide you through the enrollment steps.
In-Home Visit
After your introductory call, a member of our team will visit your home to help you complete and submit enrollment paperwork.
Medicaid & Insurance Overview
After your introductory call and in-home visit, a member of our team will help you facilitate the necessary Medicaid and health insurance evaluations of the individual needing care.
Get Authorized
Once your paperwork, Medicaid and insurance evaluations are complete, you will have to wait from authorization. Our team will stay in touch with Medicaid and insurance contacts to make sure your authorization is on track.
